MODEL OF CARE POSITION DESCRIPTION | REGISTERED NURSE

POSITION PURPOSE: 

The resident receives safe and quality care and services that meet their needs, agreed goals and preferences. Care and services are provided in a way that is culturally safe, appropriate for people with specific needs and diverse background and supports reablement.

KEY RESPONSIBILITIES

Registered Nurse (RN) practice is centred on the person, evidence based with preventative, curative, formative, supportive, restorative and palliative elements. RNs work in therapeutic and professional relationships with individuals and families. 

1. People & Leadership

  • Practices in accordance with Opal HealthCare's values, policies and procedures and all legislative requirements • Maintains own capability for practice through ongoing professional development and growth.
  • Models leadership skills and support/coach and supervise the care team, including mentorship as required.
  • Appropriately delegates aspects of practice to enrolled nurses and others, according to enrolled
  • Provides effective timely direction and supervision to ensure that delegated practice is safe and correct
  • Participate in education sessions and share knowledge with care team members and maintain and meets annual refresh and essential core education and training requirements
  • Supports induction programs and ongoing training for new and existing care team members.
  • Supervise students on placement.
  • Ensures all Agency care team members are orientated.
  • Participates in own and other team members’ annual performance/development reviews as required.
  • Uses the appropreate processes to identify and report potential and actual risk related issues.
  • Identify and report where practice may be below the expected standards.

2. Quality Care

  • Comprehensively conducts assessments that are holistic as well as a culturally informed, trauma integrated healing approach. Uses a range of assessment techniques to systematically collect relevant and accurate information and data to inform practice.
  • Works in partnership to determine factors that affect, or potentially affect, the health and wellbeing of the person to determine priorities for action and/or for referral, in line with the person’s preferences and agreed goals of care.
  • Uses assessment data and best available evidence to develop a plan of care
  • Ongoing liaison with members of the integrated team, to identify resident care needs, plan interventions and review outcomes of care.
  • Ensure that management is informed of any relevant issues and informed of any resident incidents including advice of reportable incidents within designated timeframes.
  • Follow WHS standards/safe work practices.
  • Incorporates and applies infection prevention and control principles in all work practices. Active involvement in the identification and implementation of continuous improvement initiatives.
  • Active participation in and contribution to organisational meetings.

3. Financial

  • Utilise and manage resources in an efficient and cost effective manner, through appropriate use of clinical consumables, equipment and minimise wastage.

  • Present oneself in a professional and respectful manner.
  • Ability to communicate accurately and effectively in both written and verbal forms.

  • Ability to lead a team and direct work.
  • Knowledge of the aged care industry and regulations.

EXPERIENCE/QUALIFICATIONS

  • Current registration as a Registered Nurse with AHPRA.
  • Previous clinical experience (aged care desirable).
  • Post graduate qualification gerontology or palliative care or dementia (desirable).
  • Current police check (within last 3 months).
  • Basic Life Support certificate.
  • NDIS clearance
