The Role
To carry out activities to ensure Health Care Practitioners (HCPs) and Patients are provided with high quality, safe and efficient nursing services within the Marion Medical Centre as a Practice Nurse Team Leader.
Key Responsibilities
Direct Patient Care Activities
- In the Treatment Room (or otherwise) and in accordance with company Standard Operating Procedures (SOPs) and/or Standing Orders, conduct a range of direct patient care activities.
- Direct patient care activities may include; administering vaccinations and assisting with catch up schedules, administering medications, conducting ECG, spirometry and other diagnostic tests, wound care, bandaging and plastering, removal of sutures, triage and emergency management, pre-employment medical screening and workcover injury management and set up and assist with HCP-led procedures.
Chronic Disease Management
- Assist the GP with conducting health assessments, GP management plans, team care arrangements, reviews and initiate other chronic disease management tasks (e.g domiciliary medication management reviews) in accordance with Medicare requirements.
- Provide the framework and support system to coordinate the care for patients with chronic condition(s).
- Ensure collaboration with clinical and non-clinical support teams to provide a holistic health care approach.
Nurse led clinics
- Collect health history, plan, coordinate and document patient care, and evaluate patient outcomes related to patient condition(s) and/or presentations in accordance with Primary Health Care Limited (PHC)-endorsed policies and standing orders.
- Coordinate patient care and communicate patient progress and outcomes to treating GPs and escalate any adverse findings in accordance with PHC-endorsed policies and standing orders.
- Undertake clinic administration activities to facilitate Nurse Led Clinics, as required.
- Attendance at meetings both internally and externally, where Medical Centre nursing representation is required.
- Oversight of equipment functionality, maintenance/repairs and calibration.
- Develop nursing rosters and take all possible measures to fill vacant nursing shifts.
Clinical Administration
- Clean and sterilise instruments as required and take measures to ensure instruments are maintained and in replaced when no longer in working order.
- Conduct routine activities as defined in the Treatment Room and Clinical Care Coordination checklists.
- Ensure clinical areas are tidy, clean and presentable.
- Ensure WHS policies and procedures are implemented into day to day operations
- Ensure all personnel on site, including visitors and contractors adhered to our WHS policies and procedures
- Ensure workplace inspections and audits are completed
Minimum Requirements
Qualifications
- Current AHPRA registration as a Registered Nurse (RN)
- Continuously update personal and professional skills and knowledge in order to meet the CPD requirements to maintain AHPRA registration
Experience
- Experience in a primary care setting and understanding of chronic disease management, prevention principles and health promotion, desirable.
- Management experience desirable.
