The Role

We are seeking a passionate and experienced Chronic Disease Management Nurse to join our practice who will be travelling between Blacktown and surrounding Suburbs and become a key advocate for preventative health in our local community. In this role, you'll work closely with GPs to help patients identify and manage their chronic conditions, improving their quality of life and reducing hospital admissions. The Tasks

  • Prepare Medicare compliant Care Plans to help patients manage their disease/s
  • Empower patients to increase ownership over their health
  • Coach patients on embedding health behaviour change principles
  • Enable patients to access ongoing care from a multi-disciplinary team
  • Assist in early identification of chronic disease by conducting health assessments
  • Work with GPs to promote preventative healthcare throughout the practice
  • Increase the number of patients receiving their first tailored care plan for proactive chronic condition management Ensure patient’s care plans are reviewed regularly
  • Manage administrative tasks relating to carrying out CDM appointment

 

The Successful Candidate

  • AHPRA registered Enrolled or Registered Nurse
  • Strong knowledge of chronic disease management and preventative healthcare practices.
  • Ability to collaborate with GPs and other healthcare professionals to develop and implement care plans.
  • Skilled at educating patients on managing their conditions and promoting healthier lifestyles.
  • Outstanding communication and customer service skills
  • A passion for people and a personable approach
  • Have driver's License to travel comfortably when needed in the surrounding suburbs
